Responsibilities
- Conducts initial and ongoing comprehensive assessments of the client's needs, including Outcome and Assessment Information Set (OASIS) assessments
- Ensures that required documentation is complete in a timely manner and is in compliance with regulations and standards
- Provides nursing care following established nursing service objectives and standards
- Communicates with patients and families regarding treatment and care
- Ensures that all safety and infection control practices are followed
- Performs starts-of-care, resumptions-of-care, re-certifications, and discharges
- Responds to emergency situations
- Supervises other members of the home health team
Qualifications
- Diploma in Nursing at a minimum; Bachelor's degree preferred
- Current CPR Card from the American Heart Association
- Experience in home healthcare required
