What are the responsibilities and job description for the Home Health Physical Therapist PT High Per Visit Rates position at Allegiance Home Health Services LLC?
Amcare Pro Home Health is interviewing for a Licensed Physical Therapists (PT) to add to our growing DFW team.
The right candidate would need to be a self-starter, who needs very little supervision and can exemplify the ability to supervise PTAs .
The territory is South Dallas. Previous HOME HEALTH experience is REQUIRED. Knowledge of the OASIS assessment tool would be necessary.

JOB DESCRIPTION:
- Assess and evaluate therapeutic/rehabilitative/functional status and participate in developing the plan of care.
- Direct physical therapy treatment/supervise PTAs
- Work under the supervision of a PT
- Instruct patients, families, and caregivers in the use and care of therapeutic appliances.
- Prepare and submit clinical notes in a timely manner.
Requirements of Qualified Candidates:
- Must be currently licensed to practice as a Physical Therapist or Physical Therapist Assistant in the STATE OF TEXAS and compliant with accepted professional standards and practices
- Must possess and maintain valid CPR certification while employed in a clinical role
- Valid driver’s license and reliable transportation
