Job Description:
The Hospice Clinical Supervisor (or Clinical Director) oversees the coordination, supervision, and delivery of clinical hospice services in accordance with federal and Wisconsin state regulations. This role ensures high-quality care for patients and families while supervising nursing and interdisciplinary staff, maintaining compliance, and guiding clinical operations.
Requirements:
Primary Responsibilities:

Oversee the planning, coordination, and delivery of all clinical services, including nursing, social work, spiritual care, and therapies.

Ensure compliance with Wisconsin Administrative Code DHS 131, CMS Conditions of Participation (42 CFR Part 418), and agency policies.

Provide supervision and evaluation of clinical staff, ensuring timely documentation, competency, and adherence to care standards.

Participate in the hiring, training, scheduling, and ongoing education of clinical team members.

Conduct or delegate supervisory home visits to assess care quality and team performance.

Serve as a resource to staff, patients, and families regarding clinical and regulatory questions.

Participate in quality assurance, performance improvement (QAPI), and infection control programs.

Lead interdisciplinary team (IDG) meetings and collaborate on patient care planning.

Assist with policy development, audits, surveys, and readiness for accreditation or state inspections.
- Provide clinical on-call support or emergency management when needed.
Required Qualifications (Wisconsin Specific):

Current RN license in the state of Wisconsin (required).

Bachelor’s degree in Nursing (BSN) required; Master’s degree preferred.

Minimum of 3–5 years of hospice or home health experience, including leadership or supervisory experience.

Strong knowledge of Medicare hospice regulations, compliance standards, and clinical best practices.

Excellent interpersonal, organizational, and leadership skills.
- Valid driver’s license and reliable transportation.
