Looking to hire an experienced nurse to evaluate and admit clients for Personal Care Services.
Complete and submit documentation within agency policy that is complete, legible, and clearly reflects client status.
Obtain required authorization for services via appropriate portal, following through to completion. Communicates authorization information and status with other team members to assist in coordinating care.
Position works closely with DHS RN’s, families, caregivers and other community resources on behalf of the client.
Performs all duties and responsibilities in accordance with the Nurse Practice Act and in accordance with basic principles and guidelines of professional nursing.
Applies nursing principles in the assessment of patients. Provides non-medical nursing care, emotional support and education to patients and families. Encourages patients and family members to participate in the patient’s plan of care to meet established goals.
Prefer Registered Nurse with Home Health and Medicaid Experience.
