Job Title: Care Retention Coordinator
Reports To: Ryan White Program Supervisor
Position Summary: The Care Retention Coordinator is responsible for promoting consistent engagement and retention in healthcare services, including behavioral health, medication adherence, and achieving viral suppression. This role actively identifies, engages, and reconnects clients who have disengaged from care, facilitates linkages to integrated healthcare services, and provides targeted adherence support, including enrollment and support for mobile health (mHealth) applications, to improve overall health outcomes.
Key Responsibilities:
Client Engagement and Retention:
- Proactively monitor client retention and identify individuals at risk for disengagement.
- Implement outreach strategies, including phone calls, home visits, and community-based engagement to reconnect clients to care.
- Coordinate closely with clinical and case management teams to address barriers impacting retention and viral suppression.
- Conduct outreach activities specifically targeting people living with HIV who are out of care to re-engage them in healthcare services.
Medication Adherence and Viral Suppression Support:
- Provide individualized counseling and adherence interventions to improve medication compliance, specifically targeting behavioral health, HIV care, and PrEP medications.
- Educate clients on medication benefits, schedules, side effects, and coping strategies for adherence.
- Emphasize the importance of achieving and maintaining viral suppression through consistent medication adherence and healthcare engagement.
- Assist clients with enrollment in and support for mHealth applications to enhance medication adherence and self-management.
Integrated Care Coordination:
- Collaborate closely with Psych Nurses, clinical therapists, psychiatrists, and social workers to coordinate comprehensive client care.
- Facilitate linkages to internal and external supportive resources, including medical, psychiatric, substance use, housing, and transportation services.
Data Collection and Analysis:
- Track patient outcomes and identify trends in care adherence.
- Use data to inform program improvements and optimize patient care.
Documentation and Reporting:
- Maintain timely, accurate, and comprehensive documentation in the electronic medical record (EMR).
- Track retention, adherence, and viral suppression outcomes to evaluate the effectiveness of intervention strategies and report findings.
Community Collaboration:
- Foster strong partnerships with community providers, agencies, pharmacies, and healthcare networks.
- Conduct education and outreach activities to enhance community awareness of available retention and adherence support.
Qualifications:
- Bachelor’s degree in social work, Public Health, Psychology, Nursing, or related human services field preferred.
- Professional licensure or certification (BSW, MPH, CPhT, PTCB)
- etc.) strongly preferred.
- Minimum of two years’ experience in care coordination, retention activities, behavioral health, or HIV care.
- Experience in healthcare or a related field beneficial.
- Proficiency in Microsoft Office applications, especially Excel.

- Excellent organizational, time management, and case management abilities
- Knowledge of trauma-informed care, culturally responsive practices, harm reduction strategies, and healthcare systems
- Proficiency in EMR documentation and mHealth solutions
- Empathy, compassion, and ability to connect with patients and families
Physical Requirements:
- Ability to conduct client outreach in community and home settings.
- Office environmental duties involve routine computer and phone usage.
