ACE’s CVI program serves individuals aged 14 to adult who are gang-affected or otherwise at risk of gang involvement. Through outreach activities and incident response, the program promotes community-level peacemaking and violence reduction. Through a phased program of participant engagement potentially leading to cycles of family case management services, the program works to strengthen participants' family ties and other pro-social influences.

The CVI Community Health Worker (CHW) plays a crucial role in delivering intervention services within the Community Violence Intervention program. Working in close collaboration with the CVI Lead Care Manager, the CHW supports participants by promoting health and wellness, facilitating access to healthcare, and providing self-management training under the guidance of the Lead Care Manager. The CHW is actively involved in all aspects of the CVI program, including intake, care coordination, making referrals, promoting peacemaking, and serving as a liaison within the community to support the CIW / CHW and Lead Care Manager’s efforts for the purpose of reducing the incidence of violent injury or reinjury, trauma, and related harms and promoting trauma recovery, stabilization, and improved health outcomes. This position advocates for participants to prevent injury, violence, or the onset or exacerbation of a health condition .

  • Recognizes appropriate boundaries and limitations on interactions with program participants and their families and the need to maintain professional standards and practices.
  • Understanding of Medi-Cal systems and the social determinants of health affecting underserved populations is beneficial, with a willingness to build expertise in these areas.
  • Ensures participant recordkeeping is accurate, timely, and in compliance with funder and ACE standards, maintaining confidentiality at all times
  • Cultural understanding of the marginalized West Valley communities.
  • Knowledge of gang environment / dynamics within the geographic area.
  • Provides safe passage across gang lines to gang-impacted participants to ensure the safety of participants to receive necessary services.
  • Competent in Google Suite and able to navigate participant databases.

DUTIES AND RESPONSIBILITIES

  • Fosters positive relationships with community members, partner organizations, and local gangs.
  • Participates in cultural and community-building activities and events.
  • Works to incorporate best practices for diversity, equity, inclusion, restorative justice, and trauma-informed care into the organizational culture.
  • Uses motivational interviewing, trauma-informed care, and harm-reduction techniques to help participants achieve their care plan goals, as directed by the ECM Director.
  • Provides basic health promotion, distributes health materials, and assists participants with accessing healthcare services under the guidance of the ECM Director.
  • Assist participants, parents, and families in identifying, understanding, accessing, and navigating community-based social services and healthcare providers to increase access to resources such as housing, healthcare, child care, and parenting support, Medi-Cal, CalFresh, and child welfare resources.
  • Conducts intake and eligibility determinations, conducts participant assessments leading to enrollment, and completes or updates participant service plans.
  • Arrange and participate in family meetings, service meetings, and team meetings, coordinating wrap-around services with other providers.
  • Coordinates with the LCM to ensure participants’ health and social service needs are being met, without taking on full care management duties.
  • Organizes, coordinates, and delivers skills-based workshops for participants.
  • Advocates for participants during healthcare interactions, providing pertinent information during at-risk situations or hospital admissions, as authorized by the ECM Director.
  • Monitors participants’ progress on care plans, communicating updates to the LCM  and intervention team.
  • Ensures all services are documented properly into participant databases and contributes to periodic program reports.
  • Monitors treatment and care compliance, communicating findings to the Lead Care Manager and other interdisciplinary team members.
  • Provide court advocacy and support youth accountability using trauma-informed approaches.
  • Plan and supervise field trips, facilitate wellness / healthcare activities and events, including SNL wellness activities, ensuring participant safety.
  • Other duties as assigned.
  • PHYSICAL REQUIREMENTS

  • Prolonged periods of standing, walking, and desk work
  • Must be able to lift up to 15 pounds and perform physically demanding tasks, such as bending and climbing
  • QUALIFICATIONS

  • High School Diploma or equivalent community intervention experience required
  • 1 year of experience working with at-risk populations in community or home-based settings required.
  • Formal gang intervention training (e.g., LAVITA) preferred.
  • Spanish language skills, verbal and written, preferred.
  • Valid California Driver’s License with a clean DMV record and proof of adequate auto insurance required.
  • Department of Justice criminal background check clearance required.
  • Initial TB employment clearance required.

    The anticipated pay range for candidates who will work in California is $28.85 to $30.45 hourly.  The offered pay to a successful candidate will be dependent on several factors that may include but are not limited to years of experience within the job, years of experience within the required industry, education, etc.
