We are currently hiring a full-time Consultation Intake Coordinator to join our team. This is a remote position with a typical schedule being Monday-Friday 8:00am-4:30pm. Corporate office located in Eagan.
The Department of Human Services (DHS) is replacing Personal Care Assistance (PCA) and Consumer Support Grant (CSG) services with Community First Services and Supports (CFSS). As the newest self-directed option, CFSS is designed to offer participants more choice, control, and flexibility with the services and supports they use to help them live and work in the community. Anyone who utilizes CFSS services must go through a Consultation Services Provider that is contracted with the state of MN to provide these services. The Consultation Intake Coordinator is Alliance’s first point of contact for this program and is responsible for assisting with the completion of intakes and obtaining and preparing all necessary documents to assist our Consultation Service Specialists.
Responsibilities:
- Learn and maintain current understanding of the CFSS program and be able to clearly communicate details of the program to Participants and/or Participant Representatives, and their teams.
- Complete initial intake and set-up of Consultation Services, which includes:
o Answering and/or responding to referral phone calls, emails and voicemails
o Providing a brief overview of CFSS, Consultation Services, and the intake process
o Sending out and tracking referral forms
o Monitoring, pulling, and tracking Service Agreements, Alliance Written Agreements, and Assessments/Support Plans
o Scheduling initial appointments for Participants to meet with their Consultation Service Specialists
o Entering Participant information into our EMR platform, Pavillio
· Work together with the Consult team to provide excellent participant-focused support and timely service, while executing excellent customer services skills
- Assist the Consult team with developing or carrying out new or changing policies and procedures
- Identify problems and work with the Consult team to find resolutions based on an understanding of the CFSS program requirements, guidelines and regulations
Qualifications:
· be at least eighteen (18) years of age
· be able to pass a MN Department of Human Services background check
· Experience:
1. Familiarity with disability services in MN, preferably with CFSS, PCA, or CSG
2. Customer Service, intake, and/or receptionist experience preferred
· Education: minimally have a high school diploma
Candidates must be:
· Strongly skilled in customer service and interpersonal communication, including both verbal and written communication
· Highly organized with attention to detail
· Self-managed and possess a self-driven work style
· Diligent in providing follow-up communication to Participants and their teams
· Able to work autonomously while still placing value and importance on teamwork and coordination of services and information
· Skilled at decision-making, problem solving, multi-tasking and reasoning
- Proficient with writing and reading skills
- Familiar with Microsoft Office programs, especially Excel
