OVERVIEW:
We are currently hiring one full-time field RN Supervising Professional / Qualified Professional (RN SP/QP) to join our team! In Minnesota, Personal Care Assistance (PCA) and Consumer Support Grant (CSG) services are being replaced with a new health care program called Community First Services and Supports (CFSS). As the newest self-directed option, CFSS is designed to offer Participants more choice, control, and flexibility with the services and supports needed to help them live and work in the community.
Initially, as the RN SP/QP you will essentially wear “two hats” while working with Participants with varying levels of needs in both the PCA and CFSS programs throughout the transition period. Once the state completes the full transition, your position will become exclusively focused on the CFSS program. Enjoy flexibility and a desirable work-life balance as you schedule in-home visits that accommodate your Participant’s schedules as well as your own. A small percentage of this position can be completed remotely, as needed. THIS POSITION REQUIRES THE ABILITY TO TRAVEL THROUGHOUT THE 7 COUNTY METRO AREAS OF MN AS NEEDED, WITH A FOCUS IN THE NORTH/NORTHEAST METRO AREAS.
· For PCA services – assist Participants with the development of personalized care plans; train, supervise, and evaluate PCA Workers and the effectiveness of their services throughout the service plan year
· For CFSS services – provide initial and ongoing training to Workers which may include explaining, coaching, and demonstrating tasks; observe and evaluate Worker’s competency; service delivery quality assurance.
Additional Responsibilities will include:
· Comply with intake, annual, and on-going service requirements for both PCA and CFSS programs
· Assist Participants and their families with service transitions as needed.
· Collaborate with other Alliance team members, including service coordinators and management as needed to ensure quality of service delivery
· Stay abreast of CFSS program changes and updates.
· Attend CFSS meetings or other additional meetings as directed.
· Complete any DHS or agency required training at hire and on going as necessary.
· Complete and maintain documentation as required.
· Additional duties as assigned.
QUALIFICATIONS:
Candidates must have all of the following:
· Active Registered Nurse license in the state of MN, in good standing.
· At least 1 year of supervisory and/or worker training experience, preferably in homecare
· A reliable vehicle, valid driver’s license, a clean driving record, auto insurance, and ability/willingness to travel to areas listed above
· Ability to pass a background study with Minnesota Department of Human Services.

- Be professional and service-driven to provide quality care to program Participants, Representatives, and Workers
- Be proficient with Microsoft Office applications (word, excel, outlook, etc.) and Electronic Medical Records (EMR) processes
- Possess problem solving and critical thinking skills
- Be self-managed and able to work autonomously
- Be highly and demonstratively organized
- Possess strong written and verbal communication skills with attention to spelling, grammar, and thoroughness
