Job Description
RN & LPN Community Health Care Managers are responsible for working directly with patients, families, local providers, and community agencies to provide clinical support, assess treatment plans, eliminate fragmentation of care coordination, and work to prevent gaps in care and duplication of services for members of the Medicaid population. RN Community Health Care Managers work to improve member/patient outcomes through goal setting and comprehensive care planning. Additionally, this role is critical in reducing costs associated with high utilization of health care resources while providing a patient centered approach, promoting continuity of care, and encouraging members/patients to work toward self-managed health care as appropriate. RN Community Health Care Managers report to Care Management Supervisors with clinical oversight provided by the CMO.

Knowledge / Skills / Abilities
- Strong clinical skills
- Multi-tasking, prioritizing, ability to handle multiple priorities while meeting deadlines
- Culturally competent
- Computer skills including MS Office (Outlook/Excel/Word), navigation of Electronic Medical Records, and other data collection tools as assigned
- Case Management Experience preferred
- Experience partnering with local and regional health and human service resources
- Basic business writing skills
- Ability to work independently and with limited direction
- Experience working with Medicaid and Medicare
- Bilingual Preferred
- Reliable transportation and a valid driver license and insurance
- Graduate of a Nursing Program
- Duly certified and licensed as a Registered Nurse or Licensed Practical Nurse to practice Medicine in the State of Colorado
- Current CPR Certification
- Assists Community Health Care Managers in the clinical component of care coordination for members/patients
- Identify risk factors leading to increased utilization while working toward reduction of preventable emergency room visits, hospital admissions/re-admissions, excessive therapies, etc. for member population
- Develop individualized interventions for members/patients
- Assure appropriate and complete data entry of all requests per program guidelines
- Participate in health promotion activities for member population
- Perform individual health risk questionnaires, assist with individualized care plan design, documentation and implementation of care plan activity, transitions of care coordination, and evaluation of outcomes
- Complete nurse consults including assessments, member screenings, medication reconciliation, care plan reviews, staff and member education
- Participate in home visits, telephone contacts, hospital visits, and other means of connecting with members to reach identified goals
- Establish effective collaboration, communication, and coordination among all responsible parties of an individual member’s multidisciplinary health care team
- Optimize member and family independence through education, community resources, and support
- Provide basic nursing services per plan of care; including assessment of vital signs, medication monitoring, basic wound care, and other duties as assigned and discussed with Chief Medical Officer
- Develop and maintain positive working relationships and communications with internal and external partners
