Your Role in our Company Will Be
The RN travels to a patient's home to administer their services and helps patients maintain their independence. Their duties include: Administering at-home IVs, changing dressings, cleaning wounds, in-depth assessments, overseeing patient treatment plans, As well as Updating Doctors & The Agency about their patient's health. Documentation and communication with the Agency is done through Synergy Software.
AVAILABILITY:
- Our patients need care 7 days a week, although you have flexibility in your scheduling, we do ask that you are able to work at least two-three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer)

Pay: $150.00 - $200.00 per visit

Experience:
- Visiting RN for Hospice: 1 year (Required)
- Nursing: 1 year (Required)
License/Certification:
- Current CA RN License (Required)
- BLS from the American Heart Association and/or the American Red Cross (Required)
- NSO Nursing License Insurance (Required)
- IV/Phlebotomy Certified (Required)
- Comprehensive knowledge of OASIS documentation (Preferred)
Work Location:
- Area or city close to you, with a group of patients who need care for that day
Requirements
RESPONSIBILITIES:
- Providing nurse visits to hospice patients. (home visits)
- Performing an initial, comprehensive assessment. Documenting everything & Uploading to Agency Portal
- Obtaining and documenting physician orders.
- In consultation with the assigned Clinical Team Manager, you will need to initiate and regularly re-evaluate and revise the plan of care
- Coordinating care with the assigned Clinical Team Manager, physician and other members of the home care team, informing them of significant changes in the patient’s condition and needs.
- Performing re-certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager. Completing and submitting all related documentation
- Assessing the need for the services of other team members (PT, OT, ST, MSW, and HHA).
- Performing Home Health Aide supervisory visits per regulatory requirements.
- You'll demonstrate knowledge of regulatory requirements.
