Overview
In order to ensure timely processing of the patient record, the HIM Technician will ensure receipt of all inpatient/outpatient/emergency department patient records; review each patient record for poor original, items that cannot be scanned, patient identification on each document, and will group like documents in the appropriate order. This position is also responsible for indexing all documents to the appropriate patient folder and for ensuring each document is assigned the appropriate document name. In order to allow for timely completion of the medical record, the HIM Technician will review each discharged patient's record for incomplete or missing items, identify the responsible practitioner for each of these items, and link the deficiencies to the appropriate practitioner for completion. The HIM Technician will be responsible for verifying chart completion and monitoring the reanalysis queue to ensure all records are completed appropriately. The position will also be responsible for timely notification to practitioners of pending records that need completion and identifying those practitioners who will be suspended.
Responsibilities
- Retrieves discharged medical records from various departments in the hospital and reconciles them to ensure that all records are accounted for.
- Facilitates the retrieval and printing of medical records from storage, as well as the storage, archival and record retention of documents.
- Responsible for queries, data consolidation, data analysis and management reporting utilizing software to generate metrics and develops actionable recommendations. Supports all areas of the hospital and clinic by collecting, analyzing data and reporting results.
- Works closely with providers, coders, and billers to ensure accurate procedure and diagnosis codes are being utilized in the pre-authorization process. Ensures accurate and timely processing of all insurance claims and follows up on all prior authorization rejections.
- May assist with the physician suspension process by evaluating if a physician should be put on suspension, creating the list of recommended suspensions for approval, sending out notice letters, making reminder calls, etc.
- Provides clerical support to HIM staff in relation to filing and retrieval of medical records. Assists in other department functions as assigned, such as photocopying, faxing, typing, and any other departmental task as requested by the HIM Administrator. Assist in keeping provider credential files current and in compliance with Mississippi licensure laws.
- Prepares medical records and loose documents for scanning.
- Scans medical record documents.
- Indexes medical record documentation.
- Performs a paper document to PC screen quality control validation to ensure that all documents associated with each record have been scanned.
- Special projects and other duties as assigned.
Experience
- Proficient in English, both written and verbal, for effective communication within the team.
- 3 years of HIM experience
