Full Job Description
Under the direction of the DON the home visit RN/LPN will manage and oversee a caseload of clients to deliver comfort and excellent care as required.
Responsibilities and Duties
Provide skilled nursing care to pediatric and adult patients in a home setting
Implement, develop, and document the plan of care to ensure quality and continuity of care for each patient
Provide effective communication to patient/family, team members, and other healthcare professionals
Implement plan of care and treatment using sound judgment and critical thinking
Provide accurate, detailed, and timely documentation consistent with the plan of care and the nursing process
Administer oral, IM, Subcut prescribed medications, also treatments, and therapies
Participate in staff meetings to exchange ideas and information as it relates to the quality initiatives.
Qualifications and Skills
Current Registered Nurse in the state of MA
Minimum 1 year of clinical home care experience /New grads are WELCOME AND WILL BE TRAINED
Exceptional listening and analytical skills required.
Excellent attention to detail.
Work positively and productively as part of a team.
Work flexible hours when needed.
must demonstrate excellent notes and documentation skills.
Experience in Home Health Care
Knowledge of HIPAA regulations
Work independently.
Maintain confidentiality.
Current CPR accredited by American Heart Association and/or American Red Cross (certificate must be healthcare provider or healthcare professional).
Valid MA Drivers license
