Nursing Assessment Specialist – Premier Homecare
At Alliance Homecare , we specialize in providing premier, client-centered in-home care for high-net-worth families in the NYC metro area. Known for our culture of excellence, professionalism, and empathy , we are expanding our elite nursing services and searching for an RN Intake Specialist to join our team.
About the Role :
As an RN Intake Specialist, you will be the first point of contact for new clients, ensuring a smooth onboarding process and setting the foundation for exceptional care. This role is perfect for a highly organized, detail-oriented nurse who thrives in care coordination and patient advocacy .
Key Responsibilities :
- Conduct initial assessments for new patients, ensuring a seamless transition into homecare services.
- Hold the case for the first 1-3 days , providing initial stabilization, patient education, and coordination before transitioning the case to the Registered Nurse Care Manager (RNCM) .
- Develop and implement individualized care plans in collaboration with the Director of Operations, caregivers, and medical providers.
- Serve as a compassionate and professional liaison between patients, families, and the care team.
- Ensure compliance with homecare regulations and maintain detailed documentation.
This is a Monday-Friday, 9 AM-5 PM role. Approximately 75% of your time will be spent in the field , visiting clients’ homes.
