Key Responsibilities
- Conduct functionally based assessments to identify client needs.
- Develop comprehensive treatment plans tailored to enhance functional independence.
- Instruct individuals with disabilities in vital life skills to foster greater independence within their communities, including but not limited to budgeting, grocery shopping, and self-care practices.
- Prepare meals and snacks according to specific dietary instructions.
- Accompany clients on errands and medical appointments to ensure their well-being.
- Perform housekeeping duties as necessary, including cleaning and laundry.
- Execute client-specific tasks as directed by nursing professionals, including assisting with prescribed exercises and understanding specialized care needs.
- Proficient in conducting functional assessments and developing goal-oriented plans.
- Strong interpersonal and communication abilities to build rapport with clients and their families.
- Knowledge of daily living skills training approaches.
- Willingness and capability to travel to clients’ residences for service provision.
- Master's in Social Work or a Bachelor of Arts degree in a relevant field.
- At least one year of experience working with individuals facing behavioral difficulties, traumatic brain injuries, or other disabilities.
- Valid transportation to facilitate travel to various client locations.
