Key Responsibilities
- Deliver personalized training in independent living skills to clients.
- Conduct functional assessments to develop individualized treatment plans.
- Monitor client progress and adjust plans as necessary to ensure effective outcomes.
- Collaborate with team members to provide comprehensive support and service delivery.
- Travel between clients as assigned across Nassau and Suffolk Counties, the five boroughs of New York City, and the Lower Hudson Valley.
- Proficiency in conducting functionally based assessments.
- Expertise in developing and implementing comprehensive treatment plans.
- Strong ability to teach and guide individuals with disabilities towards functional independence.
- Excellent problem-solving skills and adaptability in various situations.
- Valid driver’s license and reliable transportation for client travel.
- Bachelor’s degree in a relevant field with a minimum of two years of experience, or an Associate’s degree with three years of experience in a similar role.
- Demonstrated experience in conducting functional assessments and crafting treatment plans.
- A solid background in supporting individuals with disabilities is essential.
