The CM supervisor oversees the CM and assigns their clients to them. The CM supervisor has a minimal case load of 4-7 participants.

The Case Manager supervisor supervises the case managers. The CM supervisor, working with the Outreach Worker supervisor and the Light in Night supervisor ensures that the CP4P model is successful. The CM supervisor oversees the CM and assigns their clients to them. The CM supervisor has a minimal case load of 4-7 participants.
Essential Functions
Accomplishes participants' care by assessing individual needs; providing direct services and engaging in ancillary activities; participating in an interdisciplinary team of other PBMR staff and community partners. The essential functions of the position include, but are not limited to, the following:
- Works with the CMs and ensures that the relationships are professional and consistent to provide support and encouragement to the youth/clients.
- Supervisors the CM’s in one-to-one supervision sessions as well as group sessions.
- Supervises internal and external services being provided to participants; identifies and arranges caseloads.
- Monitors cases by verifying participants' attendance; observing and evaluating activities and responses of the CM; identifying and connecting to additional resources; intervening in crises; and providing personal support
- Supports and teaches as necessary case management training and arranges for outside trainings to better equip the CM team including case management, trauma awareness, maintaining healthy boundaries, self-care, etc.
- Maintains participants' records by inputting case notes, assessments, and events daily in online data system
- Communicates CM progress by engaging in weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to team and family, and identifying program influences.
- Helps CM to engage in participants' exit strategy by reviewing goals and individualized plans, coordinating discharge and post-discharge requirements, orienting, and training family members, and providing resources
- Develops partnerships with resources that will strengthen support for participants and holds partners accountable as needed
- Case Managers are mandated reporters and are expected to work at high level of team engagement
- Flexible schedule - this position requires some nights and weekends.
- At least 3 years of experience supporting and working with in-risk youth and street-involved youth and developing individualized plans for in-risk youth; awareness of trauma reactions.
- At least 3 years of experience working with people from various ethnic, economic, and educational backgrounds both as colleagues and as clientele.
- Experience documenting case notes.
- Experience developing and retaining relationships with other service providers.
- High level of interpersonal and conflict resolution skills.
- Ability to communicate effectively both verbally and in written form.
- Crisis intervention training or knowledge.
- Knowledge of Chicago and communities with histories of violence, experience in Back of the Yards & Englewood neighborhoods preferred.
- Proficient in Microsoft Office Suite and comfortable using computer programs to track participant engagement.
- Imaginative, dynamic, development professional with a passion for the PBMR mission.
- Bachelor’s degree in Human Services field (i.e. Social Work, Sociology, etc.) or equivalent experience.
- No pending criminal cases
- PBMR is a drug-free and safe workplace, passing a drug screening and background check is required prior to being hired
- Candidate must have their own vehicle, valid driver’s license, vehicle insurance, and good driving record; and feel comfortable engaging in community-based services
- Participate in and successfully complete the Virtus Training (i.e., Archdiocese of Chicago Safe Environment Training).
