The successful candidate will have strong Primary Care or Hospitalist experience and be able to manage health problems while coordinating care in compliance with health regulations.
Responsibilities:
- Conduct thorough assessments and develop individualized treatment plans for patients
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Maintain accurate and up-to-date medical records
Qualifications:
- Graduate of an accredited school of nursing and an accredited Nurse Practitioner Program
- Current APRN licensure in New York State in good standing
- Current DEA certificate
