Clinical Care Coordinator LPN Direct Access Job Description:
The Clinical Care Coordinator LPN Direct Access will work closely with our healthcare team to ensure seamless coordination of patient care services. This includes conducting nursing interviews to assess patient needs and develop individualized care plans.
In addition to your clinical expertise, you will also be responsible for managing patient flow, communicating with patients and families, and collaborating with other healthcare professionals to achieve optimal patient outcomes.
