We are on the search for a Registered Nurse Case Manager to join our growing Hospice and Palliative Medicine group. The Case Manager will coordinate, evaluate and plan an individualized care plan for patients who are receiving hospice or palliative care from a group of healthcare providers. The qualified candidate will have an in-depth understanding of hospice and palliative care services and have experience coordinating numerous concurrent programs at once. Responsibilities:

• Monitor patient's physical, mental, and emotional wellbeing.
• Educate patients and their families about hospice and palliative care.
• Prepare and implement a treatment plan for patients in cooperation with physicians.

Qualifications

• Current RN License with experience in home health and hospice care
• Proficient in managed care, patient assessment, and discharge planning
• Strong knowledge of medical terminology, HIPAA regulations, and physiology
• Experience in critical care, geriatrics, and hospital settings preferred
• Ability to develop and implement effective care plans for patients
• Familiarity with inpatient, outpatient, and post-acute care environments
