Summary
We are seeking a Registered Nurse - Home Health to join our dedicated team at Allied Home Health. In this role, you will be responsible for delivering exceptional nursing care to patients in their homes, ensuring their health and well-being while supporting their independence.
Responsibilities
- Provide direct patient care in home settings, including administering medications and treatments as prescribed.
- Monitor and assess patient health status, including vital signs and overall condition.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Educate patients and families on health management and disease prevention strategies.
- Maintain accurate medical records and documentation of patient progress.
- Coordinate with other healthcare professionals to ensure comprehensive patient care.
- Assist with case management activities to optimize patient outcomes.
Requirements
- Current Registered Nurse (RN) license in the state of practice.
- Experience in home health or community-based nursing preferred.
- Strong knowledge of physiology, medical records management, and case management practices.
- Excellent communication skills and ability to work independently.
- Basic understanding of managed care principles is a plus.
