Description
Under the supervision of the AVP of Admission, the Skilled Nursing Admissions Facilitator will be responsible for all the activities related to the admission of patients to our Skilled Nursing Center.
EDUCATION :
Bachelors or Associates Degree Preferred.
High School Diploma or Equivalency Required.
WORK EXPERIENCE :
Prior nursing experience in a long-term care setting required.
CHARACTERISTIC DUTIES & RESPONSIBILITIES OF JOB :
Facilitates all patient admissions including but not limited to finding beds, ordering equipment, validating insurance, assisting in precert process, completing preadmission assessments, collaborating with case managers, speaking with patients and families, maintaining accurate bed boards, maintaining accurate bed hold list as well as working with the business office. Keeps updated on all Medicare and Department of Health Regulations and is available to in-service admissions team on any changes.
Maintains positive relationships with referral sources by visiting / making his or herself available to case managers and referral sources, to answer any questions regarding current or future referrals and general inquiries. Becomes a member of professional organizations that are directly related to Skilled Nursing Facilities and care, as well as attends community functions which allow for dialogue between like facilities, case managers, community and state regulatory agencies and professional colleagues. Allow for ability to aid in marketing opportunities that will keep referral sources up to date on all programs and admission opportunities.
Attends twice daily standup meetings to discuss status of bed availability, and review all pending admissions dates and times.

Ensures financial responsibility and management by developing, monitoring and / or implementing departmental / divisional budget.
Responsible for adequate staffing levels by ensuring employees are scheduled, paid and receive time off when requested. Accurately completes and approves timecards. Monitors and evaluates department objectives to use time effectively and meet deadlines.
