GENERAL SUMMARY :
Under the supervision of a Patient Care Supervisor, the Licensed Practical Nurse administers direct nursing care to patients and performs a variety of indirect patient care duties. The License Practical Nurse is responsible for all professional working policies pertaining to this section.
The Licensed Practical Nurse must be able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on his / her unit - adolescent through geriatric. The Licensed Practical Nurse must demonstrate knowledge of the principles of growth and development over the life span; possess the ability to assess data reflective of the patient's status; interpret the appropriate information needed to identify each patient's requirements relative to their age-specific needs; and to provide the care needed as described in the Home Health policies and procedures.

EDUCATION :
Graduate of an accredited Licensed Practical Nurse Program required.
A pharmacology course of at least forty-five hours must be included in the basic education
curriculum or have been attended post graduation.
WORK EXPERIENCE :
Home Health experience preferred.
Med-surg or rehabilitation experience preferred.
Minimum 2 to 3 years experience as a LPN preferred.
SPECIAL EMPLOYMENT REQUIREMENTS :
Current licensure as a Practical Nurse in the Commonwealth of Pennsylvania.
Must maintain the standards of nursing care and implement the policies and procedures of the Home
Health division and Allied Services.
Must be cleared by PA State Police criminal record history check.
Good verbal and written communication skills and inter-personal sills required for effective
Interaction with staff and patients.
Valid PA driver's license and available transportation during all working hours required.
Must maintain confidentiality of patient information.
PHYSICAL DEMANDS :
Moderate Work : duties require exerting up to 25 lbs. of force occasionally (approx 10-33% of shift), and / or up to 10lbs. of force frequently (approx 33-66% of the time), and / or a negligible amount of force constantly to lift, carry, push, pull, and move objects (i.e., occasional patient transfers).
Position involves bending, reaching, stooping, and lifting (occasionally in strained positions)
throughout shift.
Ability to transport self throughout facility is required.
Must be able to travel to patient's homes as required.
ACCIDENT OR HEALTH HAZARDS :
Occasional exposure to blood and body substances.
Exposure to needle sticks possible.
WORKING CONDITIONS :
Works in a well-lit office environment and in varying conditions in the patient's home (where conditions may be less than desirable).
