Description
Under the direction of the Vocational Day Program Registered Nurse, the Licensed Practical Nurse is responsible for assisting in overall coordination of health care services of client participating in the Day Care and Work Activities programs of the Vocational Day Care Center.
EDUCATION :
A graduate of an accredited nursing program with current registration and licensure as a Licensed Practical Nurse in the Commonwealth of Pennsylvania.
A Pharmacology course of forty-five (45) hours must have been successfully completed or experience list below.
WORK EXPERIENCE :
One (1) year nursing experience and with aging adults and adults with functional impairments is required.
Minimum of six (6) months experience passing medications or pharmacology course listed above.
SPECIAL EMPLOYMENT REQUIREMENTS :
Sight, hearing, and speech sufficient to effectively communicate with clients.
Must pass post job offer, pre-placement and bi-annual physical examination with annual communicable disease screening, and child abuse criminal background check.
Must possess a current Pennsylvania nursing license.
PHYSICAL DEMANDS :
Medium Work : duties require exerting up to 25 to 50 lbs. of force occasionally (approx 10-33% of shift), and / or up to 25lbs. of force frequently (approx 33-66% of the time), and / or a negligible amount up to 10lbs. of force constantly to lift, carry, push, pull, and move objects.
Position involves bending, reaching, stooping, and lifting (occasionally in strained positions) throughout shift.
Position requires ability to transport self throughout facility.
ACCIDENT OR HEALTH HAZARDS :
Exposure to body substances and communicable diseases.
Position also requires client crisis intervention techniques (i.e. client restraint, redirection, etc.) as described in job duties. Individual must be particularly aware of needle sharps and infectious disposable procedures.
WORKING CONDITIONS :
Individual works primarily in a well-lit nursing station. Work is also performed in a classroom type setting with inherent distraction and noise.

- 45% Administers medication and tube feedings as ordered by physician including oral, topical, and parenteral when necessary. Insuring that the orders are current and on file. (E)
- 5% Observes the condition of clients reports these observations to the responsible living unit, nursing service, community agency, family members and takes appropriate action as prescribed. (E)
- 5% Assists the Day Program Registered Nurse in performing the duties of the Health and Safety Nurse regarding incident / accident reports, staff physicals and other safety related corporate issues. (E)
- 4% Records and reports all observed symptoms and changes in individual's conditions to the Registered Nurse. (E)
- 3% Participates in and provide in-service training and education particularly in regard to CPR- First Aid and other medical related issues. (E)
- 3% Administers PRN medications as ordered by the physician in accordance with established medical procedures. (E)
- 3% With the Day Program Registered Nurse performs a periodic review of the Day Care and Work Activities programs to evaluate their health-related effectiveness. (E)
- 2% Assists in the coordination and implementation of the health related plans outlined by each Individual Program Plan. (E)
- 2% Attends Inter-disciplinary team meeting and other staff meetings as circumstances indicate. (E)
- 2% Assists in maintaining accurate medical records and instructs personnel in both the Work Activities and Day Care Programs. (E)
- 2% Provides individual support, instruction and direction for individuals as the situation warrants. (E)
- Assists the Day Program Registered Nurse in the following :
- 8% Performs the duties of a Developmental Assistant in the absence of direct care staff and / or when programmatically indicated. (NE)
- 6% Providing assistance in the management of disruptive clients and implementation of behavioral management plans. (NE)
- 3% Assisting developmental Assistants in the provision of appropriate Program Activities. (NE)
- 2% Coordinating the lunch program emphasizing appropriate eating techniques and procedures. (NE)
- 2% Monitoring the dietary needs of clients participating in the program and provides reports to the appropriate dietitian. (NE)
- 2% Overseeing individual instruction in the area of Activities of Daily Living. (NE)
- 1% Requisitions such as supplies and equipment needed to operate a First Aid Station. (NE)
