StartDate: 3/3/2025 Available Shifts: 8 D Pay Rate: $2252.92 - $2468.62
PT- Home health needed asap for 13 weeksTuesday - Saturday. First week will be M-F to facilitate orientationPhysical Therapist needed to provide one-on-one therapy care for patients in their home setting!We are looking for a candidate with no requested time off.Primary Job Responsibilities:Assess a person’s condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence. Collaborate with interdisciplinary team and colleagues to ensure a person’s needs are met. Demonstrate ability to perform standard therapy measurement techniques, including ROM, strength, and balance tests.Utilize a variety of therapeutic modalities to maximize care provided to clientsMileage Details: IRS rate, applied for travel between patients, not for commuteRequirements:Vermont PT licensureACLSBLS/BCLSHome Health experience and OASIS experience a must as this candidate will case manageMcKesson/Netsmart EMR experience strongly preferred1 year experience
