We are seeking a Contract to Hire Patient Navigator (PN). Patient Navigator is a culturally competent non-clinical healthcare worker who supports and guides patients throughout their care continuum. PNs support patients to anticipate, address, and overcome barriers to care, while guiding patients through the delivery systems that provide their care. PNs work closely with the rest of the Health Center team to ensure high patient satisfaction.
Location San Jose, CA– You may be scheduled to work at the health centers located in Santa Clara Valley Medical Center area and / or the Regional Medical Center area.
Salary Range : $24
Day Shift : Monday – Friday - occasional evenings and / or weekends
Duties and responsibilities
Engage potential patients and schedule new patient appointments.
Conduct assessments and surveying patients to assess their needs.
Perform warm handoffs.
Enroll patients into eligible insurance programs; helping patients’ problem-solve insurance and billing issues; etc.
Assist with screening and enrolling new clients.
Provide care coordination for clients.
Provide translation and interpretation and non-technical explanations during medical visits and while making referrals.
Document and maintain records per appropriate standards and regulations.
Conduct outreach to promote the Center’s Health Center services.
Requirements
Completion of accredited Medical Assistant Program, Patient Care Representative Program, or similar training in Case Management, or similar experience in a healthcare setting.
High School Diploma or Equivalent
Completed patient navigation or case management experience, or similar.
Experience working in a health care setting.
Verbal and written fluency in English required; bilingual in a second language (particularly in Spanish, Vietnamese, Mandarin, Tagalog, Farsi) highly desired but not required.
Committed to providing high quality customer service.
Proficient with Microsoft Office applications
Experience working with Electronic Health Records
Ability to establish and maintain effective work relationships as part of a multi-disciplinary team.
Ability to communicate well with people of diverse cultural professional and experiential backgrounds and lived experience. Credentials

MMR vaccines (two vaccines / titers), Immunization Records or waiver.
Varicella vaccine (chickenpox) Immunization Records or waiver.
Hepatitis B series (three vaccines / titers) or Immunization Records (no waiver options)
TB testing within 1 year.
Current CPR Certification.
