Position Title: Nurse Navigator (RN)
Position Type: Full-time (Exempt)
Location: Travels to locations throughout Salem, Burlington and Camden Counties (New Jersey) and the surrounding area.
Job Summary:
Allies is seeking a Nurse Navigator (RN) to provide individuals in our group home settings with IDD diagnosis healthcare support with a preventative approach in mind. Nurse Navigators work independently in the community and are required to travel from home to home to provide in-person visits in Allies' group homes and day programs. Nurse Navigators see 2-3 individuals a day and provide specialized trainings to staff / individuals to ensure competency levels across the agency. In addition to home visits and physical assessments, Nurse Navigators will provide in-home trainings. Nurse Navigators support the training department and rotate New Hire training and PPD administration. Nurses participate in all individual planning meetings and work closely with behavior supports as well as community-based supports to ensure Allies' individuals become as independent as possible in the community while managing their independent medical needs.
Home Health experience is preferred but not required. A successful candidate will have the ability to work independently in the field, be collaborative with a remote team approach, be confident in managing and scheduling their own caseload, and eager to advocate for Allies' individuals to receive the right level of care at the right time to encompass equality and equity of care for everyone Allies serves.
Qualifications:
- Graduate of an accredited Nursing School
- Registered Nurse (RN) licensed to practice in the State of New Jersey
- Three years of experience working with individuals with intellectual and/or developmental disabilities (I/DD)
- Valid Unrestricted Driver's License with a good driving record
- Proof of valid auto insurance coverage
- The ability to lift and transfer at least 50 pounds
- The ability to sit, bend, squat, reach, and stand for several hours at a time
- Intermediate level Computer Literacy
- Must be able to read, write and verbally communicate in English and direct/respond to individual's needs Must be able to perform clinical assessments and procedures independently
