A large managed care organization has a position for a Case Manager LPN. It is preferred to have case management experience along with having a IV Certification. Must be fluent in written and verbal skills for English and Spanish.
- PAY : $32.00 / hr
- CONTRACT : 12 / 11 / 2024 to 03 / 14 / 2025 (Will change upon hire)
- Hours : Monday- Friday 8 : 00am- 5 : 00pm
- LOCATION : Davenport, FL 33897
- Blood test, MMR, Varicella, Hep B titers, respirator medical clearance are required screenings along with drug and background
Description :
Licensed Practical / Vocational Nurse with an active and unrestricted license to practice in the state of employment
Current BLS certification
Two or more years of experience in a physician's office, clinical or hospital setting
Knowledge of chronic diseases, especially COPD / asthma, Diabetes, CHF and IHD
Excellent verbal and written skills
Strong interpersonal skills
Ability to interact productively with individuals and with multidisciplinary teams
Proficient computer skills to work efficiently with electronic medical records
Excellent organizational and prioritization skills
Preferred Education, Experience & Skills
Experience related to patient education and / or motivational interviewing skills
IV Certification
Fluent written and verbal skills in English and Spanish
