Responsibilities :
Complete initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of in-home care Veteran clients
Assess clients’ conditions and chart observations, and perform evaluation tasks
Educate clients and their families on proper home health care strategies and procedures, and make recommendations for devices or tools that might improve the client’s quality of life
Staff Supervisory Visits
Complete appropriate and timely documentation of all client care, including progress note, summaries, and per state requirements
Provide effective communication to clients, their family members, and other team members
Qualifications :
Current state license in good standing as a LPN
TB up to date
Current CPR certification
Compassionate, friendly demeanor
Excellent communication skills
Current driver’s license and reliable transportation
