Allura provides a range of healthcare services, including the provision of Home Health services that are performed in home settings. We are interested in hiring Registered Nurse Case Managers to help run these services, ensuring that patients receive optimal care and working with our clients to complete detailed assessments and provide quality services. The ideal candidate will be able to work independently, make thoughtful decisions and offer expert guidance to patients on ways they can help themselves during their recovery process.
Responsibilities:
- Assess patients and ensure their needs are being met by health care services from medical facilities.
- Use home health data to make decisions about patient care, including discharge planning and re-admissions.
- Conduct home visits for assessment and care coordination.
- Investigate new medical information to determine the most appropriate plan of care.
- Make changes to the patient's care plan according to available resources.
