The Service Coordinator s upports the operations, programs and activities for services provided under the NYSDOH Home and Community Based Services Medicaid Waiver for Individuals with Traumatic Brain Injury (TBI) and / or Nursing Home Transition and Diversion (NHTD) Waiver.
Education & Experience
- The provider shall have at a minimum, one (1) year of experience providing Service Coordination in human services and Master’s degree.
- Be an individual with a Bachelor’s degree and two (2) years of experience providing Service Coordination in human services.
