Clinical Nurse Specialist - Home Healthcare
We are seeking a highly skilled Clinical Nurse Specialist to join our home healthcare team. The ideal candidate will have expertise in developing and implementing individualized plans of care/service plans and providing high-quality clinical services to patients in their homes.
**Responsibilities:**
- Develop and implement individualized plans of care/service plans according to federal and/or state program requirements.
- Provide high-quality clinical services to patients in their homes.
- Collaborate with other healthcare professionals to ensure seamless care coordination.
**Requirements:**
- Current RN licensure in the state of practice.
- Minimum one year experience as an RN (preferred).
- Current drivers license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification required.
