About the Role
The Home Care Licensed Practical Nurse is responsible for delivering direct nursing care to patients in their homes, as prescribed by their physician. This involves conducting thorough assessments, developing personalized care plans, and implementing interventions to promote optimal health outcomes.
You will have the opportunity to work independently, yet as part of a collaborative team, to ensure seamless coordination of care. Your expertise will be instrumental in maintaining accurate and comprehensive medical records, communicating effectively with patients, families, and healthcare providers, and addressing any concerns or questions they may have.
Key Responsibilities
- Provide High-Quality Clinical Services: Deliver direct nursing care within the scope of your practice, adhering to all applicable infection control procedures.
- Document Patient Visits: Record visits thoroughly and completely, per regulatory and payer requirements, in the electronic medical record.
- Participate in Care Coordination: Actively participate in our agency's care coordination process, including timely reporting of patient needs and changes in condition.
- Maintain Confidentiality: Ensure confidentiality of patient health information per agency policy.
Requirements
To be successful in this role, you must have current LPN licensure in the state of practice, as well as a valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation. Current CPR certification is also required.
