We are seeking a compassionate Licensed Practical/Vocational Nurse (LPN/LVN) to join our team at Almost Family - Marinette PD. As a LPN, you will provide direct nursing care as prescribed by the physician to acutely ill and/or chronically ill patients in their homes.
Job Summary
The Home Care Licensed Practical/Vocational Nurse is responsible for delivering high-quality clinical services within the scope of practice, following all applicable infection control procedures. This role involves recording clinical and progress notes, coordinating other patient care activities/services, and providing the highest quality of patient care.
Key Responsibilities
- Providing high-quality clinical services to patients in their homes.
- Documenting patient visits thoroughly and completely per regulatory and payer requirements.
- Actively participating in the agency's care coordination process.
- Ensuring confidentiality of patient health information.
Requirements
To be successful in this role, you must have current LPN licensure in the state of practice. A current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation are also required. Current CPR certification is mandatory.
