Job Description:
The Registered Nurse will provide and direct provisions of nursing and personal care to patients in their homes, as prescribed by the physician or as authorized by the state/program and in compliance with applicable laws, regulations, and agency policies.
The Registered Nurse will coordinate the plan of care with other health care professionals involved in care and help to achieve and maintain continuity of patient care by planning and exchanging information with the physician, agency personnel, patient, family, case manager, and other community resources.
Responsibilities:
- Conduct comprehensive clinical assessments and develop individualized plans of care.
- Implement evidence-based interventions to promote optimal health outcomes.
- Collaborate with other healthcare professionals to ensure seamless care transitions.
- Provide education and support to patients and their families.
Requirements:
- A minimum of one year experience as an RN is preferred.
