PRN Homecare Visit RN Job Description
We are seeking a PRN Homecare Visit RN in the Escanaba, MI area.
Job Summary
The Registered Nurse in Home and Community-Based Services provides and directs provisions of nursing and personal care to patients in their homes as prescribed by the physician or as authorized by the state/program and in compliance with applicable laws, regulations, and agency policies.
Responsibilities:
- Provides High-Quality Clinical Services: Provides high-quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care/service plan, and in coordination with other members of the patient/client's care team.
- Completes Nursing Assessments: Completes nursing assessments in accordance with federal and/or state program requirements and as required by payer. Ensures the patient/client's eligibility and medical necessity for services as defined by payer source and agency policy.
- Develops and Revises Plans of Care: Develops and revises individualized plans of care and/or service plans according to federal and/or state program requirements with other community providers. Provides oversight and supervision of the plan of care/service plan as required by agency policy, federal, and/or state program and as required by payer including services provided by the licensed practical nurse and paraprofessional, as assigned by supervisor.
- Ensures Accurate and Timely Documentation: Ensures accurate and timely visit documentation of visits and services. Adheres to the electronic visit verification program as required. Reports clinical findings including changes in patient condition to the clinical manager/supervisor and physician timely.
- Administers Medications and Treatments: Performs treatments and intervention according to proper technique and procedure. Administers medications per physician order and in accordance with federal, state, or local regulations.
- Participates in Patient Case Conferences: Participates in and contributes to patient case conferences.
- RN On-Call Rotation: Participates in the RN on-call rotation for the agency, if required.
- Ongoing Training and Education: Completes all orientation and annual training timely as required by the organization and according to federal, state, or local regulations. Assists with the orientation of agency staff as needed.
- Performance Improvement and Compliance: Participates on the organization's Performance Improvement and Compliance programs and adheres to policy and procedures.
Experience Desired:
- A minimum of one (1) year experience as an RN preferred.
- Must have current RN licensure in state of practice.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- Current CPR certification required.
State Specific Requirements:
- GA: For Private Home Care Providers: a current Georgia RN license to practice nursing and render services in accordance with the provisions of the Georgia Registered Professional Nurse Practice Act and two years' experience in home health services or a related field.
- LA: At a minimum, one year of clinical experience as a Registered Nurse. One (1) year of clinical experience as a Registered Nurse may be waived for a Registered Nurse with recent clinical experience as an LPN. RN licensure must have no restrictions.
- AR, CT, FL, KY, MO, NY, NC, OH, PA, TN, TX, VA, WI: No other state specific requirements.
