We are seeking an RN Clinical Manager for our Home Care agency in the Fond du Lac, WI area to oversee clinical operations and provide field and office support.
The RN Clinical Manager is responsible for coordinating high-quality, cost-effective patient care. Key responsibilities include managing scheduling, staffing, intake, physician communications, billing, and reimbursement processes, as well as ensuring overall compliance.

Key Responsibilities:
- Manage, supervise, and ensure high-quality, cost-effective patient care and services, including referral coordination, patient need assessment, and individualized plan development and implementation.
- Ensure effective communication with physicians, families, and other clinicians involved in care.
- Communicate with staff through orientation, case conferences, mandatory training, and regular meetings.
- Ensure that only qualified personnel are hired and competent to perform assigned tasks, including licensure and certification verification.
- Conduct in-home visits as needed.
- Oversee accurate completion of clinical medical record documentation, including electronic visit verification.
- Assist with complaint investigation, resolution, and follow-up.
- Provide oversight to the agency's infection control plan and practice.
- Serve as Emergency Management Coordinator in the absence of the Executive Director.
- Participate in Performance Improvement and Compliance Programs, adhering to policies and procedures, and developing action plans.
- Perform all other duties assigned.
Requirements:
- Registered Nurse licensed in the state of practice.
- Minimum one year of Home Care related experience preferred; supervisory or management experience also preferred.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
State-Specific Requirements:
- Category I and II Medicare Certified Home Health:
- CT: One year of full-time clinical experience in a home health agency and one of the following: master's degree, bachelor's degree with three years of full-time clinical experience, or associate degree with three years of full-time clinical experience and ANA certification or community health nursing credits.
- LA: One year of clinical experience as a registered nurse.
- KY, NY, OH, TN, WI: No additional requirements.
- Category III and IV:
- FL: At least one year of supervisory experience as a registered nurse.
- TX: Registered nurse licensed in Texas with at least one year of experience within the last 36 months.
- AR, CT, PA, GA, MO, NC, NY, PA, TN, VA: No additional requirements.
- Prior management experience preferred.
- Knowledge of home care regulations and regulatory requirements preferred.
