We are seeking a Registered Nurse Field Supervisor in the Fond Du Lac area. The RN Field Supervisor is responsible for coordinating high-quality, cost-effective patient care.
Key Responsibilities:
- Manage and supervise patient care services to ensure high quality and cost-effectiveness.
- Oversee referral coordination, patient needs assessment, and development of individualized plans of care.
- Communicate effectively with staff, physicians, and families involved in care.
- Ensure qualified personnel are hired and competent to perform assigned tasks, including licensure and certification requirements.
- Provide in-home visits as needed and accurate completion of clinical medical record documentation.
- Assist with complaint investigation, resolution, and follow-up.
- Oversight of infection control plan and practice.
- Emergency Management Coordinator in the absence of the Executive Director.
- Participate in Performance Improvement and Compliance Programs.
Requirements:
- Registered Nurse licensed in the state of practice.
- Minimum one year of Home Care related experience preferred, with supervisory or management experience also preferred.
- Current CPR and driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
State Specific Requirements:
- CT: One year full-time clinical experience in a home health agency and one of the following: master's degree, bachelor degree with three years of full-time clinical experience, or associate degree with three years of full-time clinical experience in the past five years and evidence of certification by ANA as a community health nurse or completion of at least six credits in community health nursing theory or health care management.
- LA: One year of clinical experience as a registered nurse.
- KY, NY, OH, TN, WI: No additional requirements.
- FL: Must have at least one year of supervisory experience as a registered nurse.
- TX: Must be a registered nurse (RN) licensed in Texas and have at least one year of experience as an RN within the last 36 months.
- AR, CT, PA, GA, MO, NC, VA: No additional requirements.
Skills:
- Prior management experience preferred.
- Knowledge of home care regulations and regulatory requirements preferred.
