Job Title: RN Care Manager
Location: Tarzana, CA
Position Type: Permanent
Base Salary: Up to $173,000 Benefits (based on experience)
Shift: 8-hour Variable Shifts 8:00 AM – 4:30 PM or 4:30 PM – 1:00 AM (Weekend rotations required)
Role Overview:
The RN Care Manager will work in a collaborative care management practice that includes patients, caregivers, nurses, social workers, physicians, payers, and community resources.
The role focuses on:
Effective transitional care management
Utilization management
Care coordination across the continuum of care
Addressing social determinants of health

Required Qualifications
Education:
Bachelor's Degree in Nursing (BSN) or an equivalent combination of education/experience.
RNs without a BSN must obtain the degree within 3 years of hire.
Licenses & Certifications:
Active California RN License (upon hire)
California Fire and Life Safety Card (within 30 days of hire)
Experience:
2 years in a healthcare-related field (Acute, Ambulatory, Post-Acute, etc.)
2 years in Case Management, Care Coordination, or Utilization Management OR successful completion of the Transitions in Practice (TIP) program for Care Managers.
Preferred Qualifications
Education:
Master’s Degree in Nursing or a Healthcare-related field
Certifications:
Certification in Case Management (ACM or CCM)
Salary : $114,000 - $173,000
