Responsibilities include but not limited to:
The RN Case Manager is responsible for providing nursing services within the scope of practiced set and enforced by the Texas Board of Nursing. The RN Case Manager is a part of a team that also advocates for our clients. The team meets yearly and as needed to ensure that all of our client's needs are met. The RN Case Manager will work with our Host Home Providers and Direct Care Staff in our group homes to ensure the health and safety of each client. This position requires the RN Case Manager to monitor the overall health of the clients, provide supervision and guidance to the DCS and ensure the quality of all documentation.
Duties include but not limited to the following:
- Adhere to the guidelines set forth by the Board of Nursing (BON) and the HCS Principles;
- Complete Initial Comprehensive Nursing Assessments on all new clients.
- Complete Annual Physical Assessments and update assessments as needed if issues or changes arise.
- Complete Delegations when it is determined it is needed and ensures the delegated staff receives the appropriate training and skills are regularly monitored.
- Ensure first dose of new medication is given by a nurse when medication administration is delegated;
- Complete Medication Training for all Direct Care Staff and Host Home Providers, as needed;
- Make a determination of a Consumer’s Responsible Adult (CRA);
- Provide supervision of DCS.
- Administer and monitor client’s medications;
- Monitor the client's health data and information.
- Assist in getting necessary referrals for appropriate medical services;
- Perform health care procedures as ordered/prescribed by a physician/medical practitioner as required by standards of professional practice or law to be performed by licensed nursing personnel;
- Orders medications as needed for the Group Homes;
- Ensures a 72 hour supply of all medications are on hand at the Group Homes;
- Participate in Annual and Service Planning Team meetings, as needed;
- Agrees to be on call after office hours, as needed to assist the clients;
- Responds appropriately to all consumer crises.
- Complete progress notes in a timely manner.
- Turn in billing notes in a timely manner.
- Participate in scheduled department and all staff meetings;
- Complete chart reviews of Medical Charts for Quality Assurance and to be survey ready at all times;
- Keep licenses current and in good standing.
- Other duties as assigned.
Qualifications (Certifications, Licenses, or Registrations):
- Current certification from Texas Board of Nurses;
- Current certification in CPR and First Aid;
- Dependable transportation, valid driver's license and automobile liability insurance.
Experience:
- Working knowledge of accepted nursing procedures for individuals with disabilities;
- At least one year experience in home health;
- At least one year of experience working with individuals with disabilities;
- Familiarity with HCS regulations and contract standards of the program;
- Must possess team-building skills in order to effectively work with others and provide quality services;
- Must exhibit good relationship-building skills with clients and their families;
- Computer literacy.
Job Type: Contract
Pay: $30.00 - $34.00 per hour
Expected hours: 20 – 25 per week
