Job Duties & Responsibilities:
- Receives case referrals and accepts those who the agency is capable of providing services for.
- Reviews available patient information related to case, including home visits, to determine home health care needs. Assigns appropriate home care personnel to case as needed.
- Conferences with attending clinicians regarding any questions about an individual's eligibility for services.
- Reviews and evaluates each case by reviewing the services provided by clinicians, conferences, record review, discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services, and is available at all times during operating hours to assist clinicians as appropriate.
- Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.
- Attends, runs, and ensures documentation of case conference meetings with organization personnel to facilitate coordination of care, for those services areas lacking appropriate case conference leadership.
- Makes recommendations for employment of individuals. Assists in the orientation of new organization personnel.
- Provider recommendations to Director of Clinical Services in the planning, implementation, and evaluation of in-service and continuing education programs.
- Complies with accepted professional standards and principles.
- Available on an on-call basis for after-hours consultation with clinical staff as needed. May be included in on call rotation as needed.
- Participates in public relation and community activities that promote the Organization's role as an effective member of the health care delivery system.
- Complies with accepted professional standards and principles.
- Performs 90 day and annual reviews for Nursing, Medical Social Worker and Certified Nursing Assistant personnel.
- Available on a PRN basis to assist with visits throughout Snohomish County in urgent situations which may include but are not limited to, staff illness or staff vacation coverage.
- Ensures the proper and timely completion of the Clinical Manager and QAPI workflow, reporting any findings otherwise directly to the Director of Clinical Services.
- Performs other duties and activities as delegated by the Director of Clinical Services.
- Recognizes gaps in education among clinical personnel and clinical processes and provides insight and assistance in establishing system change and/or staff education.
- Coordinates with coding personnel to ensure sufficient documentation is present for the accurate coding of the patient’s medical record.
- Attends, further develops, and leads Clinical Accountability Meeting weekly.
- Any other duties assigned by DCS or Executive Director.

Job Requirements?
- Must be a registered nurse, licensed physician, physical therapist, speech-language pathologist, occupational therapist, or social worker with current licensure to practice in the State.
- Previous experience in home care setting required and with two years management or supervisory experience preferred. Proven ability to work within an interdisciplinary setting.
- Complies with accepted professional standards and practice.
- Have excellent observation, good nursing judgment and communication skills.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Complies and maintains current CPR certification.
- Demonstrates excellent observation, verbal and written communication skills.
