**Key Responsibilities**

* Evaluate patients' functional abilities and develop individualized treatment plans* Conduct therapy sessions, including exercise programs, education, and support* Monitor patients' progress and adjust treatment plans as needed* Collaborate with other healthcare professionals to ensure seamless care coordination and transitions* Maintain accurate and timely documentation of patient progress and outcomes
