JOB SUMMARY LPN coordinator is responsible for ensuring that patient care is coordinated and managed appropriately and that the care and services rendered are life changing. The LPN coordinator is responsible for managing aspects of reviewing referrals, establishing and maintaining positive relationships with customers and referral sources, and responding to customer requests and concerns in a timely manner. DUTIES & RESPONSIBILITIES Clinically reviews Home Health referrals for eligibility; determines urgency of referral Ensures compliance with all state, federal and joint commission referral/intake regulatory requirements as well as agency policies Serves as a responder to inbound calls Establishes ad maintains positive working relationships with current and potential referral sources and fellow co-workers Builds and monitors community and customer perceptions of Alpha Home Health through all communications and relations Provides recommendations to DCS in the planning, implementation, and evaluation of in-service and continuing education Complies with accepted professional standards and principles within scope of practice and following best practice Available on an on-call basis for after-hours consultation with clinical staff at least 1 week of each calendar month. This expectation not to include in-home patients visits at any time without prior notification(s) from DCS Available on a PRN basis to assist with field visits and/or urgent situations which may include but are not limited to, staff illness or staff vacation coverage Attends Clinical Accountability Meeting on a weekly basis Participates in quality assessment performance improvement teams and activities. Conferences with Attending Physician regarding any questions about an individual’s eligibility for services Attends, runs, and documents case conference meetings with organization personnel to facilitate coordination of care as needed or directed by Clinical Manager with approval of DCS Participates in quarterly record reviews and communications findings and recommendations to Director of Nursing and appropriate organization personnel Complies with accepted professional standards and principles Provides direct care as defined in States Nurse Practice Act Implements plan of care initiated by the registered nurse Provides accurate and timely documentation consistent with the plan of care Collects assessment data and performs observation to report to and collaborate with the RN(s) and physician(s) Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, MSW, ST and Dietician) to the registered nurse and/or Assistant Director of Nursing or Director of Nursing Uses equipment and supplies effectively and efficiently Performs other duties as assigned by the registered nurse, as applicable Maintains stocking levels of supply closets and orders patient and stock room supplies, as directed Coordinates/communicates with referral sources to receive and provide verbal report on potential and current patients Assists in overseeing the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy. Participates in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs. Maintains comprehensive working knowledge of Alpha Home Health’s contractual relationships and ensures that patients are admitted according to contract provisions. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Alpha Home Health.
