- Performing and documenting physical assessments
- Assessing disease progression and managing pain
- Executing care plans under RN direction
- Completing an average of 25 patient visits weekly
- Collaborating with case managers and documenting care
- Teaching self-care to patients and families
- Maintaining confidentiality and attending professional growth meetings
- Current LPN licensure in Indiana and CPR certification
- Experience in long-term care and home health preferred
- Ability to perform physical tasks and work in various conditions
