Seeking a full-time and PRN Physical Therapist for Assisted Living communities in Ft Wayne, IN. Responsibilities include:

• Evaluating and treating patients based on physician orders.
• Conducting thorough functional assessments and developing individualized treatment plans.
• Documenting patient progress and revising treatment plans as needed.
• Instructing and supervising assistants in treatment procedures.
• Collaborating with nursing staff, patients, and families to enhance functional abilities.
