Program Coordinator Group Homes
Responsible to people supported, Clinical Services Director.
Position Summary:
Responsible for the delivery of all services for the person supported. Collaborate with management professional support staff to ensure the quality of all programming. Assist in the development of policies, procedures, and other decision making processes. Serves as an internal advocate for person(s) supported and facilitates community integration.
Key Responsibilities:
- Ensure care is delivered in a non-judgmental, non-discriminatory manner that is sensitive to the person(s) and familys diversity, preserving their autonomy, dignity and rights.
- Incorporate the philosophy and the mission statement into delivery of all services.
- Guide the development and implementation of the person(s) supported OSP.
- Ensure the OSP reflects the agencys role as supporter and facilitator of the wishes and desires of the person supported.
- Develop and maintain a positive rapport with internal and external entities that are involved in each person(s) service plan.
- Promote and provide opportunities for community involvement.
- Practice and promote active supports with all persons supported.
- Organize and review all supported persons related information.
- Review and update persons information in Therap.
- Address any issues with person(s) supported programming.
- Assure assessments and are being completed.
- Assure GER recommendations are being followed.
- Monitor staff meetings and assist in development of staff schedules.
- Provide staff training regarding OSPs, plan updates and behavioral concerns and record training documentation.
- Assist person(s) supported to obtain/maintain the benefits of federal, state, county, and private support programs.
- Follow the representative payee guidelines, if applicable.
- Produce quarterly reports of person supported progress to principal team members.
- Serve as a human rights advocate.
- Knowledgeable of administrative practices and procedures, as well as applicable regulations and guidelines pertaining to programming and standards compliance.
- Participates in CQL, HCBS, and other compliance surveys as directed.
- Member of the administrative and admissions team.
- Protect and preserve operations by following HIPPA guidelines.
- Other duties as delegated by the Executive Director.
Skills and Qualifications:
Education:
*Bachelor of Science in a human service field or an individual who holds at least a bachelors degree in a professional category specified in PI-09-09 483.430 paragraph (b)(5).
*Developmental Disabilities module certification.
Experience:
One year experience working with individuals with an intellectual or developmental disability.
Other:
- Active certification in Cardiopulmonary Resuscitation.
- Flexible and attentive to detail
- Possess excellent time management and scheduling skills.
- Strong communication skills
- Basis computer skills and familiar with electronic medical records.
- Active drivers license
- Able to pass a criminal background check and drug screen.
- Able to work independently and as part of a collaborative care team.
Physical Requirements:
Strength: Sufficient to assist with lifting and transferring a person(s), and perform CPR. Ability to lift twenty (20) lbs. Moving, lifting or transferring of person(s) may involve lifting of up to fifty (50) lbs, as well as assist with weights of more than one hundred (100) lbs.
Mobility: Sufficient mobility to bend, stoop and bend down to the floor, ability to move around rapidly and in small, confined areas.
