Program Coordinator

Position Summary
Responsible for delivery of all services for the persons supported. Collaborate with management staff to ensure the quality of all programming. Assist in the development of policies, procedures, and other decision-making processes as requested or assigned by the Director of Operations. Serve as an internal advocate for the persons supported and facilitate community integration. Develop and maintain positive relationships with persons supported, staff, guardian(s), and community involved with our facilities and persons. The responsibilities listed below are not all-inclusive as responsibilities and daily duties will vary dependent on the needs of each person supported or may include other delegated duties appropriate to this position. This position reports to: people supported, Director of Operations.
Key Responsibilities
- Incorporate our values and mission into delivery of all services.
- Guide the development and implementation of the OSP (Overall Service Plan) for each person supported and monitor the plan to ensure it reflects our role as supporter and facilitator while meeting the desires and needs of the person.
- Communicate regularly with persons supported and staff as needed; keep guardians updated as needed and involved when appropriate
- May assist or administer active care or assistance to person(s) supported when needed; assist with making medical appointments and update medications in Therap; plan recreational and leisure activities; maintain documentation and keep up on paperwork necessary for care and staying organized.
- Assist staff with, program implementation, documentation, and integration; staffing, writing objectives. Provide coaching to staff as appropriate and provide suggestions.
- Provide staff training regarding OSPs, any plan updates, and behavioral concerns.
- Review GERs, follow-up and assure recommendations are being followed, and conduct training if needed; monitor and address any issues with persons supported and assure assessments are complete.
- Assure all paperwork and reporting is completed in a timely manner, maintained, and submitted when necessary.
- Maintain knowledge of administrative practices and procedures, regulations and guidelines, pertaining to programming and standards of compliance; attend required trainings.
- Participate in CQL (Council on Quality and Leadership), HCBS (Home and Community Based Service), and other compliance surveys as directed.
- Protect and preserve HIPAA and all applicable laws.

Qualifications
Knowledge, Skills, and Qualifications
Experience and Education
As outlined in the North Dakota Department of Human Services (NDDHS): Policy Issuance 09-09 (PI-09-09), Qualified Developmental Disabilities Professional (QDDP) 483.430(a)(1)
Must have at least one year of experience working directly with persons with intellectual disability or other developmental disabilities; and at least a Bachelor’s degree in a professional category specific in paragraphs (b)(5) of the above-referenced NDDHS policy.
Completion of the Developmental Disabilities Module Certification

Minimum Physical Requirements
Strength: Sufficient to assist with lifting and transferring a person(s), and perform CPR. Ability to lift twenty (20) lbs. Moving, lifting, or transferring of person(s) may involve lifting of up to fifty (50) lbs., as well as assist with weights of more than one hundred (100) lbs.
Mobility: Sufficient mobility to bend, stoop and bend down to the floor, ability to move around rapidly and in small, confined areas.
