Job Summary
The Physical Therapist will provide skilled therapy intervention and treatment to patients in a home setting. Including, appropriate utilization of treatment plans and assessment skills. Monitor carefully the patient status and progress toward goals as well as compliance with physicians orders. Responsible for educating patients and caregivers while preparing the patient / caregiver for discharge as appropriate.
Qualifications
- Valid Physical Therapy License
- Accepting New Graduates
- Demonstrate good oral and written communication along with good documentation skills
- Must be able to travel within different areas of the Bay Area including, but not limited to, San Francisco and San Mateo County
- Must have Valid CA Driver's License
Responsibilities
Conduct thorough assessments of patients, considering their medical history, physical condition, and referral requirements.Develop individualized care plans aligning with patient needs and goals, following physician's orders and evidence-based practice.Provide direct supervision and support to PTA, ensuring adherence to the plan of care.Foster ongoing communication, provide feedback, and make necessary adjustments to ensure effective treatment.Deliver skilled therapy interventions and treatments in a home setting employing a variety of therapeutic techniques as appropriate.Monitor patient status, progress, and adherence to the care plan, making adjustments as necessary.Educate patients and caregivers on managing conditions and achieving goals, and prepare them for discharge when appropriate.Complete clinical notes, evaluations, and progress reports within the agency's established timelines, maintaining adherence to both legal and professional standards.Ensure that all necessary forms and documentation are accurate, clear, and reflective of the care provided.Attend and actively participate in care conferences with other healthcare professionals, ensuring an integrated approach to patient care.Collaborate with the care team to achieve a seamless transition between different stages of therapy and other services if needed.Actively participate in the agencys quality improvement program, suggesting improvements where necessary.Continuously strive for excellence by adhering to best practices, attending training, and maintaining up-to-date knowledge of physical therapy trends.Perform other related and assigned duties that may include liaising with other healthcare providers, participating in community outreach programs, or assisting with administrative tasks.Hours and Schedule
