What are the responsibilities and job description for the RN Hospice Case Manager position at Alphatrixx Business Solutions?
The RN Hospice Case Manager will provide family-centered comprehensive care to patients of various ages and diagnostic groups who require on-call intervention. These include specialty populations such as infusion therapy, hospice, and pediatrics. The on-call support may include clinical phone guidance, coordination of care, scheduling, and direct patient intervention through visits.
Direct Care Role:
- Provides direct care services to patients/families in community and institutional settings.
- Utilizes principles of the nursing process to assist in data collection and evaluation of nursing care.
- Utilizes effective interviewing techniques; sets limits, and collects data based on specific roles.
- Demonstrates nursing knowledge and skills in the implementation of prevention, therapeutic, and rehabilitative nursing care.
- Communicates and collaborates effectively with health care providers, third parties, patients, and families based on specific roles.
- Explains processes, procedures, and treatments to patients to gain cooperation, understanding, and ease apprehension.
- Maintains awareness of the comfort and safety needs of patients.
- Observe patients, record significant conditions and reactions, and notify the supervisor or Physician of the patient's condition and reaction to drugs, treatments, and significant incidents.
- Responds to life-saving situations based on nursing standards, policies, procedures, and protocols.
- Uses oral and written communication to convey pertinent information to members of the health care team promptly.
- Complies with company policies and procedures.
- Completes all required forms, reports, time cards, itineraries, etc, accurately and within the designated timeframe.
- Recognizes and follows established lines of communication.
- Performs other duties as assigned.
Indirect Care Role:
- Maintains knowledge of Horizon computer system for care delivery applications.
- Coordinates communications with third-party payors for pharmacy, DME, and other supplies needed for the clinical plan of care.
- Maintains knowledge of regulatory, licensing, and accreditation agencies.
- Performs other duties as assigned.
Job Qualifications:
Experience
- Minimum of 1 year of hospital experience
- At least 1 year of experience in Hospice, Palliative Care, Home Care, Oncology, and/or community health (not only LTC)
- Pediatric experience is helpful
Education
- Associate's Degree in Nursing, BSN is preferred
License, Certification, Registration
- Registered Nurse, licensed in the State of Ohio
- Basic Life Support
- Valid Driver's License

- Strong planning and organizational skills.
- Demonstrated ability to work well under pressure, within strict time parameters, and with a strong attention to detail.
- Ability to work independently with minimal supervision.
- Demonstrated ability to maintain strict confidentiality.
- Bi-lingual a plus.
