Job Summary
The primary purpose of the Chaplain - PACE role at our Federally Qualified Health Center (FQHC) is to provide compassionate spiritual care and support to patients and their families during times of health-related challenges. By offering individualized counseling, facilitating spiritual assessments, and collaborating with the healthcare team, the chaplain ensures that the emotional, spiritual, and cultural needs of each patient are respected and addressed. The chaplain serves as a vital resource, fostering a healing environment that honors diverse beliefs and promotes overall well-being within the community.
Key Responsibilities

Key Responsibilities
- Conduct comprehensive spiritual assessments to identify the needs, beliefs, and preferences of patients and families.
- Provide one-on-one spiritual counseling and support to patients, addressing emotional and spiritual concerns related to health issues.
- Respond promptly to urgent situations, offering emotional and spiritual support during medical emergencies or end-of-life scenarios.
- Facilitate support groups and spiritual wellness workshops for patients and families, promoting communal healing and connection.
- Collaborate with healthcare providers, social workers, and mental health professionals to integrate spiritual care into overall treatment plans.
- Provide culturally sensitive spiritual care that respects and honors the diverse beliefs and practices of patients.
- Organize and lead religious services, prayer meetings, and rituals according to the needs of patients and staff.
- Offer training sessions for staff on spiritual care practices, cultural sensitivity, and the importance of addressing spiritual needs in healthcare.
- Maintain accurate records of spiritual care interactions, interventions, and outcomes in accordance with organizational policies.
- Foster relationships with local faith communities and organizations to enhance spiritual support networks for patients.
- Advocate for patients’ spiritual needs and rights within the healthcare system, ensuring they receive comprehensive care.
- Engage in personal self-care practices and supervision to maintain emotional and spiritual well-being.
- Conduct home visits for participants to provide personalized spiritual support and assess the home environment, including the ability to distinguish odors that may impact well-being.
- All other duties as assigned.

Required Qualifications:
- Bachelor’s degree in religion, theology, pastoral counseling, psychology, or a related field.
- At least one year working in healthcare.
- At least one year working with Geriatric patients.
- Association of Professional Chaplains (ACP) preferred.
