Job Summary:

The Speech Pathologist will evaluate, diagnose, and treat patients with speech or language disorders, formulate individualized treatment programs.

Duties/Responsibilities:

· Evaluate, diagnose, and treat patients with speech, hearing, cognitive, or swallowing issues.

· Under a physician’s direction, provides direct patient / resident treatment based on established plan of care.

· Screens and evaluates Resident and develops appropriate care plan

· Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and / or swallowing or cognitive problems.

· Administers, scores, and interprets specialized hearing and speech tests.

· Instructs residents, families, and caregivers to monitor speech and provide ways to practice new skills.

· Completes required forms and documents in accordance with company policy and state and/or federal regulations.

· Maintains patient records in an accurate and timely manner.

· Devises special tools and treatments for the specific needs of patients.

Required Skills/Abilities:

 

· Knowledge and skills to care for patient needs of the SNF population

· Must possess problem solving skills to successfully perform job

· Excellent verbal and written communication skills.

· Excellent organizational skills and attention to detail.

· Strong customer service orientation.

· Must be able to communicate effectively and professionally with other employees, patients, and family members

· Ability to travel as needed to perform job

 

Education and Experience:

· Graduate of an accredited Masters, or Doctorate in Speech language Pathology.

· State license as a speech pathologist required.

· Proficiency with all applicable state, federal, and professional standards

 

Physical Requirements:

· Prolonged periods of sitting at a desk and working on a computer
